Job Description

The LPN provides direct patient care under the supervision of an RN or physician. They support patient safety, comfort, and recovery while ensuring accurate documentation and communication with the healthcare team.

Responsibilities

• Administer medications and treatments as prescribed

• Monitor vital signs and patient conditions

• Assist with wound care and daily living activities

• Document care in the medical record

• Report patient changes to the nurse/physician

• Support admissions, transfers, and discharges

• Maintain infection control and safety standards

• Educate patients and families on basic care needs
